Indiana Non-Emergency Cost Estimator Notice

NOTICE:

Under Indiana insurance law, you have a right to request an estimate of the amount Wellfleet will pay, or reimburse, you for non-emergency healthcare services your medical provider orders for you. You also have a right to know the applicable benefit limitations of any non-emergency healthcare services you are entitled to receive from Wellfleet. Wellfleet is required to provide the estimate within 5 business days of its receipt of your request.
